These atrophic (depressed) scars are not all alike, and recognizing the predominant type changes the treatment. Rolling scars form broad, soft-edged undulations, caused by bands of fibrosis pulling the skin down from underneath. Boxcar scars are more rounded, with well-defined edges and shallow to moderate depth. Icepick scars are small, narrow and deep, as if the skin had been pierced — usually the most challenging to treat.
